Providing care support to people with dementia through the trajectory of their disease is about an understanding of dementia and the consequences of the disease process, but it is also about developing relationships with the person with dementia and their families.

This presentation outlines the role Nurse Practitioners, in collaboration with GP’s can play for this group of people.
