Transition Care Programs are an important clinical service in every health care organisation. The programs improve flow from the acute and sub-acute inpatient settings and provide patients a greater opportunity to regain independence and to return living at home.
This forum presents some outstanding speakers with a wealth of knowledge and experience within their particular fields. The forum will cover aspects of drugs and alcohol, clinical risk and the law, nutrition, high risk medications, teamwork and building quality within programs.
