WA seminar, August 2012 – New Initiatives in Community Dementia Care

A presentation by Karen Malone –McCusker Nurse, ‘The McCusker Nurse Service’ and Sarah White– Senior Project Officer – HIP-D, ‘Home Independence Program – Dementia (HIP-D) Service’ on 21 August 2012.

The McCusker Nurse – The McCusker Nurse is an Amana Living initiative, funded by the McCusker Charitable Foundation, as a unique ‘Model of Carer Support’ for 3 years.  This free service is a ‘one stop shop’, a ‘safety net’, designed to empower carers of loved ones living with dementia.  Ideally the ‘vision’ links carers with their own McCusker Nurse from the time of diagnosis to alleviate ‘carer burden’. The service links cares to agencies and services to avoid potential gaps from opening.

Home Independence Program – Dementia (HIP-D) Service – Silver Chain’s Home Independence Program – Dementia (HIP-D) is a pilot service that aims to assist people with dementia who are living at home to remain healthy, and to maximise their independence and abilities. HIP-D is the result of a collaborative project between Silver Chain, Alzheimer’s Australia (WA) and Curtin University’s Centre for Research on Ageing. This presentation will provide an overview the development of the service model and update on the pilot currently underway at Silver Chain.
