The Hierarchic Dementia Scale-Revised is a cognitive assessment for people with dementia.  It assesses 20 different cognitive functions and can be used from the point of diagnosis through to end-of-life care.

It is particularly useful when combined with the Possibility Oriented Approach for creating person-centred care plans based not only on a person’s limitations, but taking into consideration their remaining strengths.

The WA Dementia Training Study Centre is holding a full-day Train the Trainer workshop on the Hierarchic Dementia Scale-Revised for health professionals working in acute, residential or community settings, particularly those responsible for assessing people with dementia and creating care plans.
